In an effort to provide all of our patients with the quality healthcare they expect, we have implemented a Hospital Medicine Program. This program is available to our patients who are admitted to St. Elizabeth Hospital. We have a team of physicians dedicated to caring for our hospitalized patients, ages 16 years and older. Pediatric patients are followed by a pediatrician rather than one of our hospitalists because of the specialized care required by pediatric patients. They, in turn, lead the team of nurses and ancillary staff at the hospital to ensure you receive the best possible care. We refer to the physicians in our Hospital Medicine Program as “hospitalists,” as their only focus is providing medical care to hospitalized patients.
Since the hospitalists’ practice is entirely focused on the care of patients in the hospital, they are more readily available to meet hospitalized patients’ needs. Additionally, hospitalists enable your Primary Care Physician to be more available to you in the office rather than going back and forth between seeing patients in the office and in the hospital.
Hospitalists work full-time at the hospital to provide medical care and oversee the team of nurses, technicians and other hospital staff providing care to hospitalized patients. They also coordinate consultations with specialty physicians, as necessary for the patients. Hospitalists are readily available to the patients and their family members to answer questions and discuss their care. The hospital medicine team coordinates necessary medical care after discharge from the hospital such as home health, therapy, and follow-up appointments with the primary care physicians.
Hospitalists work together with the Primary Care Physician. They may discuss any significant findings or events. At the time of discharge from the hospital, the hospitalists prepare a detailed report of findings and treatment plans that are sent to the Primary Care Physician.
