Diabetes Education Program

Living with diabetes is a challenge, but it does not mean that you have to stop living your life. The Diabetes Education Program can help you to learn how to manage your disorder and gain control of your actions that may affect the way you feel on a daily basis.

Our Diabetes Education Program, accredited by the American Association of Diabetes Educators, has been established to help you gain control of your diabetes through education, nutrition and exercise. Whether you are newly diagnosed or if you have had diabetes for years, we can provide the education and support to help you have a better understanding of how to live with diabetes. The program is recommended for all persons with Type 1, Type 2, Gestational Diabetes or Impaired Glucose Tolerance to help control diabetes and prevent complications.

Our team consists of:
• Certified Diabetic Educators
• Registered Nurses
• Registered Dieticians

 

Standard Diabetes Education Plan*

1st Visit- Initial Assessment with Registered Nurse or
Registered Dietician (approximately 1 hour)

2nd Visit- Class 1: The Basics - 2 hours with
Registered Nurse/2 hours with Registered Dietician
(approx. 4 hours)

(Covers first 4 modules – the 4 elements of control and
what the complications are)

3rd Visit- Class 2: Problem Solving, Reducing Risks
and Healthy Coping (approx. 4 hours)

Follow up at 3 months

Follow up at 6 months

Follow up at 9 months

* Please note that a plan may be altered to meet
the needs of the patient

Educational Topics May Include:
• Monitoring Glucose Levels
• Nutritional Selections
• Exercise Regiments
• Medications
• Thriving with Diabetes- A Diabetes Education and Awareness Group

 

Enrollment

Though a physician's approval is required for enrollment in the Diabetes Education Program, you can either print the online referral form and bring to their primary care physician or call to enroll in the program. Before enrollment is completed, the primary care physician will need to be notified and approve the enrollment. In most cases, insurance will assist with the cost of this program (co-pays and deductibles apply).
