Admissions, Check In, and Registration

Ambulatory and Outpatient Surgeries & Procedures

Unless otherwise notified, patients scheduled for ambulatory and outpatient procedures should check in at the Ambulatory Surgery Center located on the third floor of Medical Plaza II, 2014 S. St. Clare Boulevard.

Outpatient Lab Work
Patients coming for scheduled or unscheduled lab testing should check in at Outpatient Registration, located in St. Elizabeth Hospital at Entrance B unless notified otherwise

X-Ray, CT, MRI, Ultrasound, Nuclear Scans
Patients scheduled for X-Rays, CT, MRI, Ultrasound, or Nuclear Testing should check in at Outpatient Registration located in St. Elizabeth Hospital at Entrance B

Wound Care, Sleep Studies, and Heart & Vascular Catheterization Procedures
Patients scheduled for wound care services, sleep studies, or catheterization procedures should check in at the Emergency Department Registration located in St. Elizabeth Hospital at the Emergency Department
Entrance. 

Surgeries Requiring an Overnight Stay
Patients scheduled for surgeries which require an overnight stay should check in at Outpatient Registration located in St. Elizabeth Hospital at Entrance B.

Patients referred to St. Elizabeth Hospital for inpatient care should check in at Outpatient Registration located in the hospital at Entrance B. Room accommodation is determined by your physician according to your medical needs and bed availability.

Please leave all valuables at home or send them home with a family member. St. Elizabeth Hospital is not responsible for damage or loss to personal property or items.

Documents Needed for Registration/Admission
Your registration process will be easier and quicker, if you bring the following documents and information:

  • Insurance card(s)
  • Medicare card/Medicaid card
  • Driver's license
  • Social Security number
  • Doctor's orders (if available)
  • Authorization (HMO/PPO patients) or pre-certification document

Discharge
Patients will need to arrange for transportation home from the hospital at discharge. A patient's physician or nurse will provide discharge instructions important to continued recovery including medication information, diet needs, activity limitations, rehabilitation recommendations, and time for a follow-up visit.

Please ensure you have all your belongings when leaving the hospital. If you leave an item behind, you may call (225) 647-5000 and ask for the house supervisor.

Patients may receive a questionnaire at home after their stay at St. Elizabeth. Your feedback is important to us.
