House Calls

St. Elizabeth Physicians House Calls Program provides medical care by a licensed Family Nurse Practitioner to patients who are homebound, have difficulty traveling to and from the clinic because of a medical condition or are post hospital discharges. The program focuses on disease management, success in optimum functioning in daily living activities and prevention of acute exacerbations of illnesses.

The House Calls program should not be utilized for emergency situations. Patients or caregivers experiencing an emergency should dial 911. St. Elizabeth Physicians House Calls Program is available through physician request only.
