This Notice of Privacy Practices (the “Notice”) describes the legal obligations of Our Lady of the Lake Ascension Community Hospital, Inc. d/b/a St. Elizabeth Hospital (the “Organization”) and your legal rights regarding protected health information held by the Organization under the Health Insurance Portability and Accountability Act of 1996(“HIPAA”). HIPAA protects only certain health information known as “protected health information.”
